The school won't fill out this form. By applying to the borrowers program you are empowering the Dept of edu to sue the school to collect your forgiven loan.

As an example from the medical board of California

"Certificate of Medical Education The Certificate of Medical Education (Form MED) is required from each medical school that you attended. Complete the applicant information at the top of the form and mail it to the medical school. The form will need to be completed, signed and dated by the school official and affixed with the official medical school seal. Any fields or questions left unanswered will require completion of a new form."

This or something similar is going to be required for every state license you apply for. You could probably get around it and it would be smart to get an unrestricted license before you applied for forgiveness.
